Klaviyo Sign-up Forms help you collect customer emails or phone numbers directly from your BurgerShop storefront. You can use these forms to grow your email list, offer discount codes, collect leads, and build marketing campaigns.

To display Klaviyo forms on your BurgerShop website, you need to create a form in Klaviyo, enable onsite tracking, then paste the Klaviyo onsite JavaScript code into your BurgerShop store preferences.

Before you start

Before setting up Klaviyo Sign-up Forms, make sure you have:

• A Klaviyo account
• Access to your BurgerShop admin
• A live or accessible BurgerShop storefront
• Permission to edit Online store > Preferences
• An email list or SMS list in Klaviyo
• A form design or offer ready, such as a discount or welcome offer

Important
For BurgerShop stores, choose the manual setup option in Klaviyo when enabling onsite tracking. Then paste the Klaviyo script into BurgerShop admin > Online store > Preferences > Scripts in Header.

Step 1: Create a sign-up form in Klaviyo

Start by creating a sign-up form in Klaviyo.

Steps:

1. Log in to your Klaviyo account.
2. From the left sidebar, go to Website.
3. Select Sign-up forms.
4. Click Create form.

Klaviyo will show available form templates that you can use as a starting point.

Step 2: Select a form template

Choose a form template that matches your campaign goal.

Common form types include:

1. Browse the template library.
2. Click the template you want to use.
3. Review the template preview.
4. Click Create form.

Tip
Start with a simple popup form if this is your first Klaviyo form. It is easier to test and manage.

Step 3: Set form name and subscriber lists

After selecting a template, Klaviyo asks you to name the form and choose where subscribers should be saved.

Steps:

1. Enter a clear Name for the form.
2. Select an Email subscriber list.
3. Optional: Select a Phone number list if the form collects SMS subscribers.
4. Click Create form.

Step 4: Customize the form design

Use the Klaviyo form editor to customize the look and content of your form.

You can update:

• Form text
• Image
• Button text
• Colors
• Background
• Form width
• Mobile layout
• Success message
• Email opt-in fields
• Teaser step, if available

Steps:

1. Open the form editor.
2. Edit the form content.
3. Review the Desktop preview.
4. Review the Mobile preview.
5. Adjust the design until the form looks correct on both devices.

Step 5: Set targeting and display behavior

Targeting and display behavior control who sees the form, where it appears, and when it appears.

Targeting

In Klaviyo, open Targeting & display behavior > Targeting.

You can configure settings such as:

• Audience
• URLs where the form should show
• URLs where the form should not show
• Location rules
• UTM parameter rules
• Cart content rules

Common setup

Use Show to all audience members if you want the form to be available for all visitors.

Use URL rules if you only want the form to appear on specific pages.

Display

Open Targeting & display behavior > Display.

You can configure:

• When the form appears
• How often it appears after being closed
• Which devices show the form
• Whether clicking outside closes the form

Step 6: Publish or schedule the form

After designing the form and setting display rules, publish or schedule it.

Steps:

1. Click Publish or schedule.
2. Choose Publish now or Schedule.
3. Optional: Run Klaviyo audit if available.
4. Click Publish now.

After publishing, the form status changes to Live.

Important
A published form will only appear on your BurgerShop storefront after Klaviyo onsite tracking has been added and confirmed.

Step 7: Enable forms on your website

If this is your first Klaviyo form, you need to enable forms on your website.

Steps:

1. In Klaviyo, go to Website > Sign-up forms.
2. Click Enable forms on website.
3. Select Set up manually.
4. Click Next.

Note
Do not choose Shopify integration unless your store is actually hosted on Shopify. For BurgerShop, use Set up manually.

Step 8: Copy the Klaviyo onsite JavaScript code

Klaviyo will show an onsite JavaScript code snippet.

Steps:

1. In the manual setup screen, find Step 1.
2. Click Copy next to the Klaviyo script.
3. Keep this code ready.

This script allows Klaviyo forms and onsite tracking to work on your BurgerShop website.

Step 9: Add the script to BurgerShop

Paste the Klaviyo script into your BurgerShop store preferences.

Steps:

1. Open your BurgerShop admin.
2. Go to Online store.
3. Select Preferences.
4. Find Scripts in Header.
5. Paste the Klaviyo onsite JavaScript code into the field.
6. Click Save.

Important
Paste the full script exactly as copied from Klaviyo. Do not remove or edit parts of the code unless you are sure what you are changing.

Step 10: Confirm onsite tracking in Klaviyo

After saving the script in BurgerShop, return to Klaviyo and confirm tracking.

Steps:

1. Go back to the Klaviyo setup screen.
2. In Step 2, enter your BurgerShop website URL.
3. Click Confirm.
4. Wait for Klaviyo to check your site.
5. When successful, Klaviyo will show that onsite tracking is enabled.

After confirmation, Klaviyo forms can appear on your BurgerShop storefront based on your form display rules.

Step 11: Check the form on your storefront

After publishing the form and enabling onsite tracking, test your storefront.

Checklist:

• Open your storefront in an incognito window.
• Wait for the form based on your display timing.
• Test the form on desktop.
• Test the form on mobile.
• Submit a test email.
• Confirm the subscriber appears in the correct Klaviyo list.
• Check that the form does not block important checkout or navigation elements.

Tip
If the form does not appear, check the targeting rules first. The form may be published but limited to specific pages, audiences, or devices.
